Dear UNC-Charlotte Parent,

 

We truly appreciate the trust and confidence that you as a parent have placed in us in the unique circumstances of providing health care when students are away from the support of parents and guardians. We have a strong sense of family and community at the University of North Carolina Charlotte and want the student’s experience here to be the very best.

 

We hope that students will maintain open communication with parents regarding their health status. Often students and parents find it helpful to agree upon methods of communication before classes begin. Coming to an agreement and establishing expectations for communication fosters trust and mutual responsibility.

 

Disclosure of details or information regarding medical diagnosis and resulting treatment is regulated by State and Federal laws on Protected Health Information. The student has the option of signing an Authorization for Release of Information Form at the time of each visit to the health center. (Blanket Releases are not honored)  This release form allows the health center staff to work with parents and school officials regarding the student’s medical condition. The form would naturally be waived in a medical emergency situation.
