Immunizations Information for All Incoming Students
 

North Carolina state law (General Statute 130A-155.1) requires anyone entering college to present a certificate of immunization that documents their compliance with all required immunizations (see specific immunization requirements and exemptions to the law below).  

NOTE: Only proof of immunizations is required; it is no longer necessary to submit a medical history form in addition.

Immunizations information may be submitted to the UNC Charlotte Student Health Center in one of four ways:


1. Print a copy of the Immunizations Form and submit to a physician to complete and sign;

2. If you have a copy of your immunization records that is stamped or signed by the administering physician/health agency and includes dates on which immunizations were administered, you can simply submit a copy of these records.  Please include your name, student identification number (social security number) and date of birth on all pages submitted;

3. If immunizations information is included on high school or other transcripts, request from that school that a copy of the transcripts be sent to the UNC Charlotte Student Health Center.  NOTE: Because transcripts contain academic information, a copy of your transcripts is not sent to the health center by the UNC Charlotte Admissions Office;

Transfers:
If you have attended another college or university that also required immunizations, contact the former university's health center to request that a copy of your records (if still on file) be mailed to the UNC Charlotte Student Health Center.

    If you are unable to download the immunizations form, we will be happy to mail one to you. Simply email your request and mailing address to Immuinfo@email.uncc.edu or call 704-687-4619.
     
  • Other Available Immunizations (costs subject to change)

    Hepatitis A: protects against this serious liver disease, which is spread by close personal contact and by eating food or drinking water containing the virus.  Recommended for persons traveling outside the U.S. to countries with high rates of Hepatitis A.

    Hepatitis B: protects against this serious liver disease, which may result in liver cancer.  Hepatitis B is spread by contact with blood and bodily fluids.

    Hepatitis A/B combination: protects against both Hepatitis A and B.

    Menactra/Menomune:
    protects against the most common strains of Meningoccocal Meningitis.  Recommended for first-year students living in the residence halls.  

    Polio:
    protects against this now rare, but highly contagious virus, spread by contact with contaminated feces or oral secretions. The virus can cause paralysis of the muscles in the chest, leg or arms in severe cases.

    Typhoid:
    protects against typhoid fever, an infectious disease characterized by fever, abdominal pain and rash.  Recommended for persons traveling outside the U.S. to countries with high rates of typhoid fever.

    Pneumoccocal pneumonia:
    protects against these strains of pneumonia.  Call for availability.

    Influenza/flu:
    protects against this acute, contagious respiratory infection.  Offered seasonally.
