Do I need an appointment?
For your convenience, scheduled appointments
are highly recommended. Specialty services and clinics require an appointment.

How much will my visit cost?
For students enrolled in the University
student health insurance plan, there are no charges.*
Student health fees, which are paid at the same
time as tuition, cover general medical and physical therapy services and a
variety of over-the-counter pharmacy medications. For those students not
enrolled in the University student health insurance plan, there are charges for
radiology and lab services, prescriptions, specialty clinics, and immunizations.
Payment for services may be paid in cash or check at time of visit or placed on
your tuition account for payment at a later date.
Once the charges have been placed on your tuition account
they must be paid at the Cashier's Office or online. Charges that appear
on the tuition account will not list what the appointment was for, they will be
listed as a "Health Center Charge."
* For students enrolled in the University student
health insurance plan, prescription charges in excess of $500 per year may be
placed on your tuition account .
Will my visit appear on my school
record?
No! All visits are strictly confidential and are
not part of your academic record. Information in the medical record
may only be shared if you sign an authorization to disclose
protected health information. No one can access your medical records
without your written permission, including University faculty and
administration, future employers, roommates, friends, or parents.

Does the Student Health Center accept
medical insurance?
The Student Health Center is happy to file claims
for all students enrolled in the University student health insurance plan.
If you have private health insurance, a
statement detailing the services you received will be provided so that you may file a claim with
your
insurance carrier. Payment for services may be paid in cash or check at time of visit or placed on
your tuition account for payment
at a later date.
Are prescription cards accepted?
The pharmacy accepts more than thirty
prescription cards. You may check with your insurance plan to see if
UNC Charlotte Student Health Services is an approved provider. Present your card at the time of your visit. If you are a Medicaid/Medicare participant, you must
present your card at the time of pharmacy services. Note: For those
students enrolled in the University student health insurance plan, prescription
costs up to $500 per year are covered.
How do I send or obtain a copy of my
medical records?
All medical records are considered confidential. If
submitting a copy of your medical records to us, please mail or hand deliver
those to:
UNC Charlotte Student Health Center
Attn: Medical Records
9201 University City Blvd
Charlotte, NC 28223-0001
* Sorry, faxes are not accepted, as transmissions can often be illegible.
This also helps prevent multiple pages from being separated. Hand
delivery ensures our receipt of your records and provides a valuable opportunity
to review the records in person for accuracy.
Students who wish to request a copy of their medical records must sign an
Authorization for Release
of Medical Information stating the reason for the request and
what person/agency is to receive the information. Release forms take 5 - 7 business days to be processed.
Only documents originating at Student Health Services may be released.
Information pertaining to alcohol or other drug dependencies and HIV test
results require a release specific to that information. All releases must
be signed by the patient. For further information, call 704-687-3980.
